Our team understands that it’s not always easy for our customers to leave home to pick up their prescriptions. That’s why we offer FREE Prescription Delivery. Our prescription delivery service is available Monday through Friday with no delivery charge. Why is charting your blood pressure important?

  • Charting provides a “time-lapse picture”
    Your healthcare provider will want an accurate picture of the situation inside your arteries. One measurement taken at the doctor’s office is like a snapshot. It tells what your blood pressure is at that moment. Since there are no symptoms for HBP and no way to sense fluctuations in blood pressure, measuring is the only way to get the facts. Readings can vary throughout the day and can be temporarily influenced by factors such as emotions, diet and medications. A record of readings taken over time can provide you and your healthcare provider a clearer picture of your blood pressure. It can be like a time-lapse picture or movie, providing information on what is happening with your blood pressure over time.
  • Charting can help eliminate false readings
    Some people experience anxiety when at a doctor’s office, which leads to temporarily higher readings. This condition is known as “white-coat hypertension.” At the other extreme, some individuals have normal readings in a professional’s office but elevated readings outside the office. This condition is often referred to as “reverse white-coat hypertension” or “masked hypertension.”

Such false readings can lead to over-diagnosis or misdiagnosis of HBP. Our charting process provides blood pressure readings over and period of time and delivers a more accurate picture of your true blood pressure.

Medication histories are important in preventing prescription errors and consequent risks to patients. Apart from preventing prescription errors, accurate medication histories are also useful in detecting drug-related pathology or changes in clinical signs that may be the result of drug therapy. A good medication history should encompass all currently and recently prescribed drugs, previous adverse drug reactions including hypersensitivity reactions, any over-the counter medications, including herbal or alternative medicines, and adherence to therapy.

Medication history errors, such as omitting drugs erroneously, are common and often have the potential to harm patients. Hypersensitivity reactions are often poorly documented or not explored in detail, which may lead to unnecessary avoidance of a drug. Accurate documentation of concomitant herbal or alternative therapies is rare, despite the importance they may have in causing adverse effects or drug-drug interactions. Polypharmacy, specific drugs, and clinical specialty can affect the risk of medication history errors.

There are various strategies to reduce medication history errors. Pharmacists are better at taking an accurate medication history than many physicians or nurses. In the context of acute hospital admissions, they reduce error, the risks of adverse drug reactions, and prescription costs. Electronic prescribing may reduce transcription errors, but it can facilitate other errors and still depends on an accurate medication history.
